E-LSP setup

Yes.

No. In fact the downstream assigns a different label and therefore you will have "n" E-LSPs, each carrying only one PHB. 

Unless the new request has the same LSP-ID and the Modify_FLAG set, which means that a change in the LSP is requested. In fact I think the authors of "draft-ietf-mpls-crlsp-modify-03.txt" draft may want to mention in their draft, that Diffserv TLV may also be modified.
